Quad/Graphics Case/ Disease Management Nurse in Sussex, Wisconsin

Quad/Graphics is seeking a highly skilled and passionate individual to join our Patient Advocacy team as a Case/Disease Management Nurse who will offer services to our employees and their families that have acute and complex health care needs as well as to members with chronic conditions at risk for poor health outcomes.

Patient Advocacy Nurses provide care management services through telephonic contact with patients/family, key medical providers, insurance vendors, ancillary service providers, other departments in the company and appropriate community resources. This role is a hybrid - it requires case management skills to coordinate services, manage care transitions, navigate insurance/benefits and streamline complex care while maintaining a disease management focus, providing education and support regarding chronic disease self-management and preventative health maintenance. The ideal applicant must have not only the case and disease management knowledge but also the patience and desire to support our members and the creativity to help grow the Patient Advocacy program.


  • Identifyat-risk and high-risk members from claims data who would benefit fromoutreach which requires working withvariety of patient ages and conditions/diagnosis

  • Utilize goal-setting and motivational interviewing to assess patientneeds, then educate and empower members to manage theirdisease(s), using accepted standards of care as a guide

  • Collaborate with thepatient/family and inter-professional team to ensure theyunderstand our health care benefits as defined by the Plan and how to maximizethese benefits to get the best care possible; assistin problem solving with provider, claims or service issues

  • Function independently, prioritize workload, organize workflow, andmanage multiple responsibilities with little supervision

  • Understand key business drivers and usethis understanding to maximize the impact patient advocacy has on ourmembers and prove return on investment

  • Provide recommendations and assist in thedevelopment of program goals and objectives for the future


  • HighSchool Diploma/GED required

  • Graduate from an accredited nursing program ADN required (BSNpreferred)

  • 5 years of experience as an RN required

  • Current and valid license in the state of Wisconsin or another compactstate license required

  • Disease Management experience required

  • Case Management experience (preferred)

  • Coaching/motivational interviewing experience preferred

  • Previous experience with data collection techniques and methods ofanalyzing and reporting data preferred

  • Exposure to/experience with project management a plus

  • Knowledge of Microsoft Office, including Word, Excel, Outlook, andPowerPoint required
