Quad/Graphics Care Coordinator for Chronic Condition Management in Sussex, Wisconsin

Quad/Graphics is seeking a highly skilled and passionate individual to join our Patient Advocate team as a Care Coordinator for Chronic Condition Management to assist in the collaboration of care for those patients with chronic medical conditions and/or complex care needs.

This position will serve as a resource and contact for patients and families in need of help navigating the health care system. Patient advocates coordinate care among multiple providers, connect patients and families to appropriate community resources, act as a liaison between providers and insurance vendors, provide patient education, and maintain appropriate communication to meet the needs of the patient and their family.

Responsibilities:

  • Coordinate services (home health, DME, etc.) as requested by the patient or their family

  • Educate members on disease processes and provide resources to better manage their diagnosis

  • Utilize goal-setting and motivational interviewing to improve patient health, using accepted standards of care as a guide

  • Work with the patient to ensure they understand their health care benefits as defined by the plan and how to maximize these benefits to get the best care possible

  • Function independently and prioritize workload, managing multiple responsibilities with little supervision

Qualifications

  • High School Diploma/GED required

  • Graduate from an accredited nursing program ADN required (BSN preferred)

  • 5 years of experience as an RN

  • Current and valid license in the state of the position or compact state license required

  • Case Management experience (preferred)

  • Disease Management experience required

  • Coaching/motivational interviewing experience preferred

  • Knowledge of Microsoft Office, including Word, Excel, Outlook, and PowerPoint.
